Incidents and Near Misses
Report an injury, a near miss or something that isn't safe, check RIDDOR reportability, and follow each report through to closed
On this page
The incident register is your record of everything that went wrong, or nearly did, from the first report through investigation to the corrective actions that stop it happening again.
Near misses and not-safe reports are free
All three kinds of report are free to file on every plan, Core included. Nearly hurt and Something's not safe are free to read too. An injury record, and managing reports (correcting them, follow-ups, the RIDDOR clock and the investigation), are part of Premium.
How do I report something?
Click Report something on H & S > Overview or H & S > Incidents. Crew use the same button on My safety. Choose what happened:
- Someone was hurt — any injury, however small. It goes in the accident book.
- Nearly hurt — nobody was hurt, but it could have been.
- Something's not safe — a hazard to put right, like a loose step or a missing guard.
The short form is in Report something. Every report gets its own number automatically.
What does the injury form ask?
When you choose Someone was hurt, click Carry on: someone was hurt. The injury form adds the injury and RIDDOR questions.
Incident Details
Choose the type — Accident, Near Miss, Dangerous Occurrence or Work-Related Illness — and enter when and where it happened (for example "5th Fairway" or "Maintenance Shed"). Write a short summary, then a full description of what happened.
People Involved
Enter who's reporting it, the injured person if there was one, and any witnesses.
Severity & Impact
Set the severity — Minor, Major or Fatal — and record days lost to work, whether medical treatment was needed and whether they went to hospital. Say whether the injured person was an Employee / Staff member or a Member / Visitor / Public.
Investigation Details
Set the status and, if you've started, who's investigating, their notes and the root cause.
Corrective Actions
Click Add Corrective Action for each thing that needs doing, with the person responsible and a due date. Then click Create Incident Report.
Stick to the facts
Incident reports can be read by insurers, auditors and the HSE. Write down what happened, where and when — not what you think caused it. The root cause belongs in the investigation.
How does TurfWise help with RIDDOR?
As you fill in the type, severity, days lost and who was hurt, TurfWise checks whether the incident looks reportable under RIDDOR. If it does, a RIDDOR Reportable Incident Detected panel explains why and how long you have to report it, with a link to the HSE's reporting page.
Once you've reported it to the HSE, tick RIDDOR Report Submitted and enter the RIDDOR Report Date and RIDDOR Reference Number.
TurfWise doesn't report to the HSE for you
The RIDDOR check is guidance based on what's on screen. Reporting to the HSE is a separate step you must do yourself.
If an incident is reportable and hasn't been sent yet, its page says RIDDOR Report Required — or RIDDOR Report Overdue once the deadline has passed.
How do I update an incident afterwards?
Things change after the day — someone stays off work longer than expected, or the investigation turns something up. Open the incident and click Edit.
The Correct incident page lets you update what happened, who was hurt, how many days they've been unable to work, the investigation status, who investigated and the root cause. The RIDDOR verdict updates as you type, showing Reportable to the HSE, Not reportable or Cannot be decided from what is recorded. Changes are logged against your name.
Come back when someone's still off
Whether an injury is reportable often depends on how long the person is off work, which you won't know on the day. Update Days unable to work as time goes on and watch the RIDDOR verdict.
How do I use the incident register?
Open H & S > Incidents. The register lists every report, newest first, with its kind, status and where it happened. The tabs across the top filter it by kind, each with a count: All, Hurt, Near miss, Not safe and RIDDOR.
Search by what happened, place or person, and filter by status and severity. The page also shows the oldest reports still Open, Where they happen, and Same place, same kind when the same thing keeps happening in one spot.
Reports move through four statuses:
- Reported — logged, not yet looked into
- Investigating — someone's on it
- Actions required — the cause is known and corrective actions are open
- Closed — done
The RIDDOR card counts reports still to send to the HSE and how many days are left. When follow-ups are switched on, a Follow-ups switch sits beside the tabs. See Safety follow-ups.
What counts as a near miss?
A near miss is something that nearly caused harm but didn't — a golf ball landing next to someone, a slip on a wet step, a tractor that nearly rolled. Logging them is how you spot a hazard before it hurts someone.
Reporting a near miss
Anyone with a staff, manager or admin login can report one, on any plan. It's deliberately quicker than an injury report: no severity or injury details.
Open Report something
Managers: click H & S, then Report something at the top of Incidents or Overview. Crew: click H & S, which opens My safety, then Report something.
Choose Nearly hurt
The form asks What happened? Pick Nearly hurt ("Nobody was hurt, but it could have been"). For a hazard that needs putting right, pick Something's not safe instead.
Say where, when and what
Fill in Where (for example "6th tee"), check When, and write what nearly went wrong under Tell us what happened. For a near miss you can also pick What sort of thing, such as Near Miss from a golf ball, Slip, Trip or Fall or Machinery. That one's optional.
Send it
Click Send report. It's saved under your name with its own number, and goes straight into the register.
Your team can also report near misses from the TurfWise mobile app. They appear in the same register.
Reviewing near misses
Open H & S > Incidents and choose the Near miss tab. Click a report to open its full record. Crew can read the team's near misses, but not change them, on My safety.
